Is it possible to go for a h1b extension from outside US

My h1b was valid from 2007 to 2009 and I had to come back to India because of some personal problems. I have never applied for an extension since I thought I would go back for a new h1b as advised by my HR manager.
Now, they are willing to transfer the H1b in my new company. When do I need to apply for the h1b transfer?

Would the 6 years count from the time it was approved or the last h1 status used?

You would still be eligible for applying a new h1b visa.
