H1B Visa Stamping "conditionally approved"

I went to the consulate interview, and they "approved my h1b visa stamping conditionally". The reason for this is because they need to verify I have not violated the SEVIS. I am a little confused as to what this means, and what can be examples of SEVIS violations.

 

More details about my case:

- My H1B petition was approved but my Change of Status was denied ( I think this was because I had to leave the country while my application was pending. I

-My Opt ends in Feb 2016 ( so I think I can technically keep working on my OPT status of my H1B change of status was denied)

- I just checked with my university, and they said the SEVIS system by error put my sevis as "Completed", while it shouldn't because my change of status was denied.
